香港的人口老化、慢性疾病問題日趨普遍、醫療服務需求日增，為香港的醫療系統帶來重大挑戰。為此，政府正努力優化醫療系統，確保它能長遠持續發展，繼續保障市民健康；而加強基層醫療是當中的首要一環。「基層醫療」是市民接受醫療服務的第一個接觸點，包括多種與保健和預防疾病有關的服務，以及社區醫護服務。政府期望通過整全的基層醫療系統，令市民對個人健康思維作一個範式轉移，促成大家求診習慣的轉變，由以治療為主轉變成加強預防疾病，長遠提升香港整體市民的健康水平。
